In addition to completing the form(s) below, it is the client's responsibility to view and familiarize oneself with all spa policies.

All clients must complete and submit the appropriate form(s) prior to the scheduled service(s). Please click on the link(s) for the appropriate form(s) for each type of service you will receive. If completing multiple forms, it is recommended to open them in separate tabs simultaneously so that you can Copy + Paste some of the required fields that are the same across all forms. New forms must be completed with up-to-date information on an annual basis by all clients. It is the client's responsibility to inform the therapist at every visit if there are any changes to the information recorded on your most recent form. Forms are secured by Jotform and can only be accessed by Lisa Parnell, owner of Lotus Organics Day Spa. Data will never be shared for any reason without client consent. Listing an Emergency Contact on the form authorizes Lisa Parnell to contact the listed person regarding any medical emergency or well-being concern.
